to display an ad during a YouTube video that contains a link to your squeeze page?
Do I need an Adsense or an Adwords account?

It was Charley who did that I believe. If memory serves you need an Adwords account, but I don't remember the specifics. You might PM Charley, or hopefully he'll stop by soon and see this thread.

It is called a Call to Action Overlay. Before it becomes available for any particular video you need an adwords account and you need to run an ad campaign to the video where you want to place the overlay. There is no minimum spend, though, so you can set your price so low that you won't have any traffic.
Once you've set up the paid campaign go back to your regular YouTube account. You'll see in your video manager a place where you can set up the call to action overlay ad.
