I'm considering the free CD strategy but I'm unsure how to integrate this with my Paypal buttons.
Since shipping and handling would cost more if it's outside of the UK, I can't do a single flat rate.
Paypal gives me a dropdown, where I can add different countries but the dropdown looks kind of small and my concern is that people will click the order button, without having selected their shipping destination.
The screen shot is not for the free CD thing by the way, just to show you what I mean, in terms of the dropdown.
I had been meaning to offer CDs for sale but haven't quite figured it out for the same reasons.

There is no screenshot in your post FYI.
But in my opinion, the only way to make free for shipping and handling work is to keep shipping to a minimum. The idea is that your handling fees cover at least some of the costs of the sale. I would start by only doing it in the UK where you can use a cheap, flat rate. If you want to do it in the US, I would try to find someone willing to drop ship for you, so that you could send a couple hundred CDs over to the US in one lot and then have them go out from there, to keep shipping costs low. Otherwise it will likely defeat the purpose, in my opinion.
