There are plenty of places out there but i have not used any of them. Even saw some people on fiverr.
Just google "music transcription service".
You ca search "transcribe music" in quotes on fiverr as well. You have to dig a bit more though. Here was one that jumped out: http://fiverr.com/derock/trans.....e-of-music

Hey John-
Great idea with google - that's given me some great links! I was googling "make sheet music from originals" and that was not working as well. I wonder why! 
And great minds think alike! I searched fiverr before I posted, but I didn't find any sheet music ones- they all seem to be lead sheets or chord charts (similar to the one you posted), but I'm going to try emailing some of those people to see if it might be something they can do anyway.
I'll keep everyone posted here re: what I do. I know a lot of musicians use Finale to play their music through midi and it makes the sheet music.
